Authorisation for Document Collection at the Language Centre
If you are unable to collect a certificate or a language proficiency certificate from the Language Centre office yourself, you can give a proxy to a person you trust.
Please download the PDF form with Adobe Acrobat, fill it in on your PC, print it out, and sign it yourself.
The authorised person will receive the course certificates listed on the form upon presentation of a valid identification document (usually student ID, identity card/passport or driving licence).
